Face and Neck Lifting

There are increasingly many so called face- and neck-lifts (face- and neck-tightening) that are based on bygone techniques but still  being used also by less experienced surgeons – the problem here is that all “soft” – bio- S- and similar lifts have a temporary effect  only because they exclusively or nearly always rely on skin tightening. Since skin is rather elastic, its tension diminishes rather  quickly with time and the initial effect fades away fast. After such an operation a face often appears rather unnatural, sometimes even  like a mask.

The surgical procedure that promises a long-lasting natural success and a naturally appearing face- and neck-tightening, is the  vertical SMAS-face-neck-lift. Here, not only the skin is prepared free but also the underlying muscle-connective tissue layer, the so-called SMAS (superficial-muscular-aponeurotic system) is tightened. This tissue layer is responsible typically for age appearance, such as deep nose-lip-folds, flattened skin contour and sagging neck skin. These age-related contours become naturally smooth again by way of inner tightening of this layer. The natural mimicry remains intact. And the face looks refreshed, younger, more vitalised and attractive.

The surgical procedure is carried out usually in the day clinic of the surgery in local or full anesthetic, and upon the patient’s preference, also in the hospital. First, the face will receive sub-dermal injections of the tumescence solution. This then, if needed, leads to liposuction through special, very fine needles. This technique facilitates the subsequent separation of the facial skin from the underlying muscle-connective tissue layer in a bloodless way. Moreover, it accelerates the skin separation and also bears less risk. A long-lasting effect is an improved tightening of the skin, especially in the neck region.

After this, fine incisions are made hidden in the ear region and to some extent also behind the hairline followed by loosening the facial and neck skin from the muscle layer. Then the SMAS is tightened – the decisive part of the operation. Finally, the superfluous skin is removed and fine stitches close the various wound layers.

One day after surgery, the wound dressing is changed and the wound drain behind the ear is removed. Already 2 days after surgery the wound dressing is taken off and hair may be washed and air-dried. The fine sutures around the ear are taken out one week after surgery and a week later the sutures in the scalp-hair region.
